Screening Identification of the causative organism before therapy is commenced permits selection of specifically targeted antibiotics. It is also important that ICUs should have a surveillance programme for antibiotic resistant organisms so that empiric treatment can be selected based on known sensitivity patterns and can be altered immediately following the identification of a new resistance pattern of any organisms on a unit. Infection control in intensive care units Microbial colonisation of critically ill patients is a prerequisite in many conditions for the development of subsequent sepsis. This process is facilitated by the use of invasive techniques including urinary and intravascular catheters, and ventilation equipment. The oropharynx and gastrointestinal tract are important sources of aerobic Gram-negative bacilli which may cause various infections including ventilatory associated pneumonia32. The skin in comparison is a primary source of organisms, including the CNS, which are the main cause of intravascular catheter related infections33"34, bacteraemia and septicaemia35. Prevention strategies for catheter-related sepsis have been produced34 and include the use of catheters impregnated with antimicrobial agents. Nosocomial outbreaks of multiple antibiotic resistant bacteria have been associated with failures in infection control. For example, contamination of ventilation equipment was shown to be related to an outbreak of acinetobacter36. Following surveillance, the organism was detected in output ducts of a ventilator machine despite the use of bacterial filters. Nelson, Cameron J. Koch; The University of Pennsylvania, School of Medicine, Philadelphia, PA, USA Introduction: Hypoxic tumor cells are resistant to radiation therapy and are the source of aggressive recurrent tumors. Human gliomas are a heterogeneous mix of oxic and hypoxic cells. EF5 is a 2-nitroimidazole that is phosphorylated and trapped in the cell in the absence of oxygen, thus providing a marker for tumor cell hypoxia. EF5 binding can be quantified from the tumors following resection, providing a direct measure of hypoxia. We utilized EF5 to measure the in vivo degree of hypoxia in a series of human gliomas and correlated this with time to tumor progression. Materials and Methods: Fourteen patients with radiographic evidence of newly diagnosed gliomas were identified and given EF5, 21 mg kg, 24 hours prior to craniotomy for resection of the tumor. At the time of surgery, an Eppendorf electrode was inserted directly into the exposed tumor to directly measure tissue oxygen levels in 11 of these patients. The percent tumor cube reference binding was calculated for the 14 tumor specimens. Recursive partitioning analysis RPA ; classification was performed for all patients prior to treating with conventional radiation therapy. Patients were followed with MRI scans every 3 months to the time to recurrence of the tumor. Results: WHO grade IIIII gliomas 3 patients ; exhibited modest hypoxia as measured by the Eppendorf 2.5% oxygen or greater ; . No grade IIIII tumor had less than 2.5% oxygen. The majority of WHO grade IV tumor cells 11 patients ; had 2.5% oxygen or less, with regions of severe hypoxia 0.1% oxygen ; seen in most of these tumors. The percent of EF5 binding was determined for all 14 patients. High EF5 binding was associated with a decreased time to tumor recurrence with P 0.02. EF5 binding correlated with RPA classification and was able to distinguish the indolent high-grade gliomas, with a long time to recurrence, from the aggressive high-grade gliomas, with a short time to recurrence. The Eppendorf measurements were not able to predict time to recurrence. Discussion: EF5 binding can directly measure in vivo glial tumor cell hypoxia and correlates with RPA to identify aggressive behavior of gliomas within a single WHO grade. This EF5 measurement can give a prediction of time to recurrence for individual tumors despite WHO grading and albenza.
